How To Get an Evaluation/Consultation

Please complete the Patient Self-Referral Form. You will be contacted by a Referral Specialist to collect the additional information required for confirming your appointment.  You may also ask your primary care physician to complete the referral process on your behalf. Simply ask them to contact us or visit the “Providers” section of this website. If you have any questions you may contact a Referral Specialist (843) 797-3636 extension 207.
